Let's not forget that chaos doesn't actually mean that your thoughts must be chaotic. Many demon lords have surprisingly rigid structures beneath them, realizing that the trappings of law are quite useful in the spreading of chaos, which can just as easily be entropy as it is madness. But... that's "true" chaos, where the Slaadi roam. Chaosi is different in the realm of mortals.
I find it interesting that most people look at the chaos in CG and CE so differently; CG is someone who believes in self-determination, while CE is supposed to be pure insanity. It's the same... the CE being simply always has its own self-interest at heart. If, for example, a stronger presence of Law in a city would further the CE's own agenda by making life difficult for his adversaries, he would not hesitate to support it.

The laws are also confusing on sigil so question is what is evil? and what is a law breaker?
As for red wizards, Drow, Necromanciers, Lich's, devils, Demons. They are predictably evil, trust is a foolish thing. So the fear/scare factor is rather low, to me evil haflings are more frightening they have an easier time getting trust.
There is a cost to evil, a known evil will either get followers who wish to prove they are MORE evil/twisted/vile or Will seek to take out another evil to prove their power. Characters will talk crap, avoid, or ignore evil.

Sigils laws are roughly set up in vague concept of peace... each Hardhead deciding on their own which it is... good side is that there's no playing around the laws. <,<

Actually, I'd say he has rather high charisma modifier and Prestige Class similar to the Dashing Swordsman, thus allowing him to use his charisma modifier for everything, as long as he comes up something sardonic to say.

house I would put at Lawful Neutral.
He is organised, likes results and cares more about solving the problem then emotions. While he cares little about the people he does care about the results. He drives his co-workers nuts because he is miserable and likes them to be miserable, it also allows him to solve MORE problems which is what he is interested in.
While his pranks, words, or actions could be considered evil it is more of CG type of actions due to they break the mold yet are for the greater good.
If he was evil, he would enjoy the suffering around him, be happy his co=workers are struggling to keep people alive, perhaps even gloat that he figured it out yet will watch them die. Then would actually seek to further his friends problems instead of seeking resolve through sarcastic and rude methods. When one of the patients dies he would not seek the answer out to save the next one he would instead mock his co=workers and put up pictures of her to remind them of their failures.

He's one of the difficult ones to pin an alignment on. I've seen equally compelling arguments for Chaotic Good. He despises regulations, does things however he feels, and while he may not be particularly friendly his goal is always the well-being of the patient, even if they aren't very happy about the way he reaches his destination (see also: Good Is Not Nice) and the solving of the "puzzle" of the illness.
