Been looking at the map posted in the lore Section and Was Wondering about which planes were connected to Sigil.. Ive heard from many the primes Were not connected all the time.. as people can slip into sigil from them..
Based on the map it looks like Sigil is connected to all the planes of Conflict, and the only way from them to the primes would be through the astral conduit.
Can a dm.. or Designer please Explain the way this works?

In short: Sigil has portals to just about every plane and layer.
Portal locations, spots are not always static, portal keys might varie, some portals are yet to be found. Sometimes portals are not two way portals, they can be just one way.

While Sigil is not a prison by any means, it isn't too farfetched that you entered through a one way portal. It's rather common thing that players pick the "I came from Prime, but can't find my way back" background admittably.

Elmister, Nystul, Raistlin... Who cares? I have no use for any prime berks.
- Raesph Athair, proving that planars can be clueless too.
Portals - those blessed little holes in the fabric of the planes. They're a thief's best friend, a planewalker's livelihood, and a record-keeper's nightmare.
- Addean, a tiefling merchant.
I would also like to add how some portals require different keys on the different sides of the single portal. Some portals even consume their portal keys. While few even shift their locations and destinations. Thus a clueless getting caged in the cage, is not that far fetched, they're clueless after all. :D

As you can see, there are some listed "known" portals, keys, destinations, and special notes on how they work. Saddly the numbers from one to four didn't come up that good with the scan.
